New Patient Registration

Anyone in England can register with a GP surgery.

You do not need proof of address or immigration status, ID or an NHS number.

If you live within our practice area you are welcome to register with us. 
 

A map of our practice area can be found on the above tab but as a guide we cover the following areas:

Puddletown and surrounding villages, including Tolpuddle, Affpuddle, Briantspuddle, West Stafford, Woodsford, Moreton, Tincleton, Bockhampton, Stinsford, Piddlehinton, Piddletrenthide, Alton Pancras, Plush, Dewlish, Cheselbourne, Melcombe Bingham, Ansty and Milborne St Andrew

 

Registration Forms

If you reside within the practice area and wish to register with us, please click on the following links to access downloadable forms for your completion.  Every individual will be required to complete and sign a GMS1 form as a minimum, plus one of the specific forms for either adults or children.  When you have completed all of the details, bring the forms to the surgery. You will be asked to sign the form to confirm that the details are correct by providing some photo identification and proof of address.

Adults please complete both forms: GMS1 form and Adult Registration Form

Children, both forms: GMS1 form and Child Registration form

Temporary patients

If you are temporarily living in the area (for up to 3 months) and need medical treatment you can apply to register as a Temporary Resident, enabling you to take full advantage of all our medical services. Temporary registration forms are available from the Reception desk at the surgery.
